Why am I getting random combo breaks in Oramond Combinatorics quest?

I really like the Combinatorics task from Oramond Quest, but I'm consistently random getting combo breaks. At first, I've considered it could be me losing focus, but the Server Log looks alright. Here is an excerpt without my heals/received damage:

As you can see, I've killed a glooth anemone first, then a devourer, just as requested. What could be the reason for this? Am I missing something?

Answer 1

Walking over the tile at the bottom of the ladder taking you into sewers resets the current quest kill count. Say you have Golem 1/1 Devourer 1/1 and Anemone 0/1. When you step on that tile, all get reset to 0/1.
