Today I saw this on a Guild: 

And I want to know how is it possible to add an "emoticon" in the "title set to" in a member of a guild, as shown in the image above.

Yeess!! I wanna have a title on my name with a funny kappa hahaha  but idk how do it.

This can be achieved by adding an addon to your internet browser, for example Global Twitch Emotes.
From their description:

"Global Twitch Emotes is a simple extension that finds and replaces all Twitch.tv emote phrases with their appropriate icons. It supports Kappa, global, subscriber, BetterTTV, and now FrankerFaceZ emotes everywhere!"
 

This is why Kid Kappa gets an emoticon next to his name after the addon is installed.

Keep in mind that this is only for the browser and his name will not look like this in-game

Ahhhh lol, his name is Kid "Kappa"  and I though it was only Kid and the " Kappa " was his Guild Title lol.
