Exactly location for the boss Old Widow on all respawns possible?

This big spider appears where? I know that apparently you can kill it on Grizzly Adams task but also it appears in Plains of Havoc somewhere... But where are the places where we can finish her off?

 The Old Widow respawn on:

• From task of Killing in the Name of... Quest, the teleport is located on the Plains of Havoc, here.

• Can appear in Plains of Havoc.

• She appear with others  Giant Spiders.
