Whats the type of damage of the headshot spell of Doctor Marrow?

Looks like Earth Damage, but im not sure about it.

The attack from Doctor Marrow is an Earth AoE attack. Mages and lower level paladins should use magic shield and possibly Sacred Tree Amulets to help negate some of the damage.

The damage is Earth, you should use earth protection and firebomb on floor to mitigate damage and fear
