The Exaltation Forge is only avaliable on Adventurer's Guild, which can be accessed by reaching a city main temple and using an adventurer's stone inside. Also the Forge looks like this: It's located on base floor at north of the Adventurer's Guild (reachable through the east room of the building) here:
