Where I can find Exaltation Forge?

I am looking for locations of Exaltation Forge- where is it located exactly ? Can I find few of them in different locations?

Any pic and map pic with location marked will behighly appreciated.

The Exaltation Forge is only avaliable on Adventurer's Guild, which can be accessed by reaching a city main temple and using an adventurer's stone inside. Also the Forge looks like this:
 

It's located on base floor at north of the Adventurer's Guild (reachable through the east room of the building) here:
