What is the Burial shroud used for?

Question

Once you complete the mission 'Drain the brain' of the feaster of souls quest, you create this item:

You see a burial shroud (protection death +5%). It weighs 5.00 oz.

It is used to enter the souls respawn in Port hope, but..

• Do you need to complete the drain the brain mission to be able to enter this respawn, or only wearing the armor allows you permission?
• Do you need to wear it every time you enter the respawn?
• Once you are inside can you take it off?

As far as I know we have to wear Burial Shroud every time we enter the spawn even if we have finished this mission before, but we don't have to wear it when we are already inside this spawn. To hunt Souls or run though the respawn and kill the bosses we can change armor.
