So, with the confirmation of Cobra loot through ugly monsters in Iksupan many questions arose for me:

Why are there ugly monsters in Iksupan?

Why do they drop Cobra items, is there any chance it's just bad design?

What are the ugly monsters themselves, where do they belong or how are they made?
 

But, without straying too far from the main question, I want to know if the appearance of ugly monsters in Iksupan is a real clue that could further explain the lore of both places and whatever Galthen was trying to achieve.

As you mentioned both places are related to Galthen. In the Lore of Iksupan the say they suffered an assault but they don't mention by whom the were assaulted so maybe and this is 100% an assumption, Galthen took the information of Iksupan and their gold to Scarlett and she decided to assaulth iksupan.

Uhm, I think that's a lore inaccuracy, Scarlet is known by "knowing" our movements even before we face her, if that's not enough, the whole order of the cobra lore is about her being cursed in her crusade against the evil magic, ugly monsters should be an actual treat for them, not a consequence, so I can't say is impossible, but I doubt that makes sense, even I tough I read somewhere that beside she wanted to leave the building something restrained her, I will ask about this on tibiasecrets cuz i don't remember.

Update: I got confirmation that indeed even if Scarlet assaulted Iksupan, Ugly monsters at Cobra are a consequence of that, so they went after cobra bastion with Scarlet not the other way around.

Also it's likely that whatever lead Scarlet to Iksupan had nothing to do with Galthen due the cobra curse and instead it has more to do with the robbery consequence of the money and blood lust that the curse itself throw at them.
