I wonder if it's possible to make totally new fansite item for any fansite (when website have own item for years)? Of course, what I mean is that the previous item will be completely changed as well as every item in the game. It's really interesting because I never heard of a similar case.

This has never happened before. The closest thing that comes to mind is the case of Dread Doll - when a bug made the newly introduced fansite item spawn on the floor, so CipSoft had to transform the existing items into something else and re-introduce the fansite item. So as you can see, technically it's possible and if CipSoft wants they can change the name, sprite and actions of existing items as they please.

In practice, I guess it's very unlikely that this will ever happen. The fansite would have to convince CipSoft to change the item with some very good reasons, since the items are chosen by contest and approved by CipSoft before implementation. Simply saying "I don't like it anymore" or "I had a better idea" definitely wouldn't be enough. Fansite items have a lot of value for their owners (monetary or sentimental) and arbitrarily changing them is not something CipSoft would do.

I've never seen a fansite item being totally remade, however I've seen the following changes:

  • Equippable: Some fansite items represent equippable items such as amulets and shields, however they might have been implemented as ordinary, non-equippable items. Upon request by the fansite admin, such fansite items may be made equippable. Examples: Jade Amulet and Journal Shield
  • Maximum text length: Upon admin request, the maximum text length that can be written on a fansite item may be increased. Example: Heavily Bound Book
  • Weight: If a fansite item is implemented with no weight, it will receive a weight value sooner or later. Example: Jade Amulet
  • Sounds: In order to fix typos, sounds - the messages triggered by using the item - may suffer small modifications.
  • Effects: According to Pochwalona, effects have been added to fansite items after their implementation. Example: Midnight Panther Doll
  • Description: In the beggining of 2014 (or late 2013), the description of all fansite items was changed in order to mention the fansite URLs. When a fansite's URL changes (e.g from tibia.wikia.com to tibia.fandom.com), the item's description should be changed accordingly. Example: Heavily Bound Book
  • Name (possibly): It is possible that a fansite item suffer small changes to its name such as capitalization of proper nouns, changing capital letters to lowercase letters (for common nouns that were wrongly capitalized), and adding/removing/changing its article.
  • New frames/animation sprites: When the graphics team improves item animations, they might modify and/or add sprites to fansite items. Examples:

You can add that it is possible to add some effects after using the item (it was done with Midnight Panther Doll in 2015). Also, a Memory Box is a good example of changing the look of the sprite. However, it is still a small visual change, not a change of the whole concept.

there is one such case on the rookie website but i don't know if it can be considered. the page had one fansite item but it was lost so it was created again and years later they got a new item.

Do you have anything that could prove that those two sites were owned by the same owner? I know that "Rookie.pl" left the fansite programme in November 2012 and "Rookie.com.pl" joined the programme in February 2016. Considering that you could still obtain the Noble Sword, original and replica, I don't think that you could be right in this case.
