Once used the Scroll of Ascension disappear?

Pretty much like the main question, I want to know if the Scroll of Ascension will disappear once used?

Hello! 

No, the  Scroll of Ascencions doesn't disappear once you use it, it only have a cooldown for 1 hour to use it again to change your appearance into a  Demon or in rare occasion to a  Ferumbras.

As a curious fact, if you get into the water (Like gray island, or any beach) the Scroll will get wet and it also gives you a brief cooldown, which shows the following message: 11:46 The scroll could get wet, step out of the water first.

I have never used the scroll but I did a quick search on wiki and it shows this item: 

You see a scroll of ascension.

It weighs 8.00 oz.

It is inscribed with faintly glowing ink and sealed with the letter F.

Notes

Used Scroll of Ascension. You need to wait an hour before using it again.

I am thinking it has a 1 hour cool down on it.
